Annual Plan

Our in-office membership plan is designed for cash patients who want simple, affordable preventive care throughout the year. The annual membership includes radiographs, cleanings, and a comprehensive exam. An additional $150 covers one prophylaxis for the year.

Referral Rewards
For every new patient referred, the referring patient receives $100 off treatment planning or any accrued fees, up to $100 per referral. There is no limit to the number of referrals or rewards a patient can receive.
Disclaimer
The membership fees (for cleanings/prophys) of $200 and $150 for preventive services are separate from any other discounts. In other words, the referral credit and the 5% prepayment discount do not reduce the membership cost for radiographs, cleanings, exam, or prophylaxis.
